Specifically, I would like a graphical tool that can help in the construction and maintenance of rail models / migration.
I have never seen visualization to actually be edited or designed, but graphs of the current setup There is one to attract. Has the has_many relationships included in the model that is defined in it. This is called Rail ERD (Entity Relation Diagram for Rail on Ruby):
